The A1c data in the Lancet article shows equivalent average A1c's which decreased from about 8.8 to 8.1 for the 2 groups (inhaled and injected). The inhalation protocol was before each meal + 1 injection ultralente before bed. The injection protocol was 2 to 3 injections a day.
While these are seemingly equivalent protocols, the main points for me are:
1) Neither of these comes close to what one can achieve with an insulin pump.
2) There is an unknown element of risk involved in the inhaled insulin which would not be justified given its lack of benefit in controlling blood sugar.
