You bring up a good question. This is how it has been explained to me from Dr. Faustman. The second component was needed in the mice to keep the so-called "cure" permanent. When she tried just using the first component (ie..CFA or BCG) the mice stayed insulin free for 40 days. Now, we have no idea what is going to happen in the human. Remember, mice live about 1 1/2 years so, 40 days is significant.
They expect that the second component will be needed but, we do not know if spontaneous regeneration of islets will occur in the human (however there is more and more evidence that this is possible from multiple sources). We do not know if spontaneous regeneration will occur in everyone? some people? It may depend on how long one has had diabetes? And, if we are lucky enough to see regeneration occur with successful dosing of BCG to eliminate these autoreactive T-cells, we do not know how long one can stay in that state. These are all the important questions that have the potential to be answered based on this projects ability to successfully measure the killing off of these auto-reactive T-cells.
