A question being asked by many academics, regulators and others is what degree of benefit from islet transplantation equates with what cost? This assumes no safety issues ( e.g. no immune suppression)
This is easy if the result of the transplantation is complete and permanent insulin independence. The cost of a life time of T1D is about $1 million, and this would be saved.

It is not as clear if the result is something less

One definition of modest success could be
1) attaining a HbA1c of less that 7%
2) lowering insulin dose by at least 20%
3) major lessening of severe hypoglycemia.
4) reduction of swings of blood glucose by at least 50%

for at least 2 years.

I have not seen any surveys done among people who have diabetes of what their opinions might be.
